It’s OK To Ask For Help
In an effort to provide a transformational continuum of care for our SHM patients, we are offering, through our strategic partnership with Christian Resource Center and Forward Foundation, Mental Health and Wellness services as part of our patient primary care plans.
Through our referral services, our compassionate teams will help provide ways to work with stress, depression, anxiety, grief and loss, chronic pain, and traumatic stress disorders that you may be experiencing.

We care about you. Persistent and recurring symptoms may signal it’s time to ask for help. If you’re experiencing any of the following for two weeks or more, consider asking your primary care provider at your next visit with us for a referral to our mental health services:

Difficulty sleeping

Appetite and weight fluctuations

Struggling to focus

Losing interest in hobbies or activities

Inability to carry on with daily routines

Suicidal/self-harm ideation
