How to Receive Services

How do I qualify to be a patient at SHM?
You can be seen at SHM as long as you do not have health insurance. Please arrive at our Monday night clinic at 3:45pm to
be seen for the first time.
Do I have to be a certain age?
We care for patients of all ages as long as they do not have any form of health insurance.
Can I have insurance?
No, we do not accept any form of health insurance at Samaritan.
If you have health insurance, you do not qualify for services at Samaritan.
What is the cost?
We ask for payment based upon a sliding fee scale, which is determined by your income and family. You will be financially screened on your first visit to the clinic and we will determine your sliding fee scale level at that time.
What do I bring with me when I come to the clinic for the first time?
Please bring identification, proof of income, and proof of residency. Valid ID includes a Texas driver’s license or identification card. Acceptable proof of income includes previous year’s tax return, paycheck stub, or letter from employer. Proof of residence includes an electric, water, or other utility bill.
What services do you provide?
We provide acute and chronic care, dental, mental health, and pharmaceutical assistance.
When are you open?
Primary Care (new patients, walk-in clinic): Mondays 3:45 p.m. to 8:00 p.m.
Medication Refills: Mondays 4:00 p.m. to 6:00 p.m.
Primary Care (established patients, by appointment only): Tuesdays 8:00 a.m. to 1:00 p.m., Wednesdays 8:00 a.m. to 3:00 p.m.
Dental (by appointment only): 1st, 2nd and 3rd Tuesdays, 5:00 p.m. to 8:30 p.m.
Mental Health: 2nd Tuesdays, 5:00 p.m. to 8:00 p.m.
Where do I get my medications?
The clinic offers a variety of free generic prescriptions to patients from the on-site pharmacy. For those requiring brand name medications, the clinic assists patients in applying for free medications from prescription assistance programs at pharmaceutical companies. Proof of income is required, and qualification is based on need. If approved by the pharmaceutical company, patients will receive 3-month supplies of medication for one year. Medications dispensed at the clinic may be refilled on Mondays from 4:00 p.m. to 6:00 p.m.
